I think single reviewer siates are generally minor, everything else is moderate except fallen angel, JERR and probably some others I've never heard of. It might also pay to say which genres they cover.

Most publishers will send your book themselves to at least one review site, Vamp. After that I think it's just a question of contacting some of the ones that take your fancy, explaining who you are and (briefly LOL) what your book's about, and see if they want to review it for you. If they say yes, you can usually just send your pdf file to them and they do the rest. :)

The first thing is often to check which ones are used by the publisher routinely. Then you have to hunt around the others sites to find the 'go to' person for sending books for review.
Next time I do this I will see about listing the actual emial addy and info required for each here.
